How do I handle many small debts?

Over the years I have racked up medical debt at several different hospitals and practices. I have really bad credit currently. I'd like to pay this debt back in such a way that it improves my credit. It's typically under 1000 dollars each, but at various establishments. Is it possible to get a loan to consolidate many small debts, rather than a few large debts? What is the best way to approach repaying them so that it will have maximum impact on my credit score?
